Please complete the change of details form and bring into Reception along with proof of the address or name change (proof not needed for telephone or email changes) for the changes to be updated onto your medical record. If you move outside the practice area you may need to find a doctor in your new area. There are instructions on what to do when you move to a new area on the back of your NHS medical card.
