To provide high quality, patient focused, primary healthcare closer to home in a responsive, supportive and courteous manner whilst ensuring all staff and patients are treated with dignity, honesty and respect.
We will do this by working in partnership with our Patient Participation Group (PPG), Primary Care Network (PCN) of local GP surgeries, Community Health and Social care colleagues and within the best use of NHS resources.
