How to register

At present we are only registering patients who live within our practice boundary. You will need to complete our registration forms and provide us with some proof of identity and residency.

As a quick reference we cover the following villages: Beckley, Brede, Brenzett, Broad Oak, Brookland, Camber, East Guldeford, Icklesham, Iden, Peasmarsh, Playden, Rye, Rye Harbour, Rye Foreign, Udimore, Winchelsea and Wittersham, but please check with our reception team for the latest information.

You will be given a named GP but you are welcome to see any Doctor you wish. Patients who have a preference to be treated by a certain GP should express their wish in writing to the Practice Manager. Where possible their preference will be met.

We invite all new patients to attend for a New Patient Health Check with one of our Practice Nurses and if you are on any regular medication you may also need to see a Doctor.  Please bring any evidence of medication you are on with you.

Please complete the forms below for any patient wishing to register:

GSM1 form

Rye Medical Centre new patient form

Communication consent form

You will need to bring these forms along with your proof of ID and proof of your address into the surgery. Anyone who is aged 16 or over needs to come in person to register. Anyone under the age of 16 can register without coming in person, but ID will still need to be shown by the person registering the child.

Once the registration forms have been checked and ID and proof of address verified the Receptionist will make a New Patient Check appointment for anyone registering over the age of 5. Patients will then be registered onto the system usually within 2-3 working days.
